F-1 STEM OPT Changes
(Change of employers & material change of I-983)

This survey is for students currently participating in the 24M STEM OPT extension to report change of employers and any material changes (including but not limited to remote work location, employer’s name, address and or site information, EIN number, supervisor’s contact information, etc.) to the Form I-983 as required by the Department of Homeland Security.

To ensure your SEVIS record is accurate and to remain in valid visa status please be prepared to submit the followings if you have changed employers:

1) Confirmation of last day of employment from previous employer
2) Final evaluation on student progress from previous employer (page 5 of the Form I-983),
3) New Form I-983 training plan from new employer

Please also report any changes of your local address via your SEVP student portal within 10 days.
